To register for access to the certificates website, please fill in all of the fields marked with an asterisk (*). After submitting this form, your information will be forwarded on for approval. In 24-48 hours you will receive your login information via email at the address you provide. Thanks for participating in our Certificates and Recognition program!

Certificates and Recognition Program Terms & Conditions

  • Download originals from this website
  • Only one person should be in charge of making and distributing copies
  • Please use your discretion on the amount copied and the distribution of certificates
  • The reverse side of the certificate must include the schools official stamp to be valid
  • The stamp must be a colored stamp of your organization or school name.  The stamp should not be black.  Any other color is acceptable.  This will help identity illegally made certificates
  • Please fill out the certificates and coupons completely, including the drive-in location
  • Each certificate must be signed and dated by the principal of the school or director of the organization
  • Only one certificate or coupon per student or teacher per visit will be accepted at the drive-in
  • Certificates will not be valid without the stamp, student info, signature and date
  • Email the monthly totals to acccentonkids@azrsi.com the first Monday of every month
